Just fill in the Your Rollover Form and send it to us with your proof of identity to Russell SuperSolution, Reply Paid 3279, SYDNEY NSW 2001. You’ll see your super in shape in no time!

Complete and return your form by 30 October 2009 and you’ll go in the draw to win either a $1,000 Coles Group & Myer Gift Card or a $1,000 contribution to your super – it’s up to you.
